Please fill out ALL the Registration and Medical Forms completely.

Then mail the forms with your cheque for registration & the $50 volunteer fee (unless you commit to and sign-up for a volunteer position at one of the fundraiser events this year) to the Registrar at the above address.

SCMHA will be hosting checking clinics prior to league play.
